Data Accumulation
Data from electronic health records (EHR), health information exchanges (HIE), and other clinical systems is translated using HL7® industry-standard formats, ensuring providers and care managers can see when patients are admitted to a hospital, transferred to another facility, or discharged through admission, discharge, transfer (ADT) notifications, and see the continuity of care documents (CCD).
Real-time Data Updates
Data is real-time and can drive user alerts, increasing adherence and compliance. Providers can submit care management requests from their EHR to be fulfilled in our TruCare platform. Care teams can see member health events in real-time so they know the member's location and can respond proactively to manage transitions in care.
