Get Practice and Patient Support with ZyterHome Assist
ZyterHome Assist is an optional, complimentary service that is included with every ZyterHome implementation. It covers everything a provider practice needs to enroll patients, as well as personalized patient support to make remote patient monitoring a seamless, successful experience. Best of all, ZyterHome Assist enables clinical staff to keep their focus on providing quality patient care. There is no additional charge for ZyterHome Assist, but provider practices must opt in to use it. Opting in gives Zyter permission to contact practice patients who have been selected to enroll in ZyterHome.

Remote Patient Monitoring and Medicare
Provider practices using ZyterHome may be reimbursed for remote patient monitoring services by Medicare using CPT Codes 99453, 99454, 99457 and 99458.

How ZyterHome Works
The pre-configured ZyterHome medical device is shipped to the patient’s home after the patient signs an electronic or written consent form. Outfitted with a simple on/off button, ZyterHome medical devices are easy to use for patients of all ages, and no technical knowledge is required. Because the devices are 4G-enabled, there is no need to connect them to Wi-Fi. All the patient has to do is turn on the ZyterHome device and follow the simple instructions in the user guide. Patient data captured from each device is transmitted to the provider’s ZyterHome dashboard for review and tracking.
Managing care plans for patients becomes more time efficient, since providers can quickly access and review all home monitoring device data on the ZyterHome dashboard.

The ZyterHome provider dashboard helps physicians manage patient care plans more efficiently with quick access and review of all home monitoring device data. Plus, physicians receive notification of alerts of abnormal patient data values so they can intervene quickly. If a physician thinks it is necessary to speak with a patient to discuss their health, they can initiate a virtual office video call through the dashboard. The ZyterHome dashboard also tracks the time physicians spend analyzing RPM data.
