Improving Care and Outcomes for Multiple
Conditions with Remote Patient Monitoring

Sept 10, 2021

Remote patient monitoring (RPM) is the 21st century office visit, and as we’re learning, a potential lifesaving modality of care. Zyter is at the forefront of RPM with our pre-configured LTE-enabled wireless devices that transmit vital sign data from a blood pressure cuff, glucose monitor, thermometer, pulse oximeter or digital weight scale.

While these devices may be fairly common and familiar to most people, when coupled with RPM they become potentially lifesaving tools not only for conditions like hypertension and diabetes, but for other health conditions adversely impacted by blood pressure, glucose levels, and weight that need to be controlled.

More and more physicians are finding that they can extend their use of Zyter RPM to provide remote care for specialized conditions in certain patient populations that previously have not had access to the tools for participating in their own healthcare. Here are some of the ways that Zyter RPM is helping patients stay compliant with treatments, as well as reducing the risk and prevalence of debilitating chronic diseases.

Hypertension Remote Monitoring

Nearly half of the adults in the United States have hypertension and only one in four has it under control. Understanding risk factors like being overweight, tobacco use, too much salt intake, race, age, and family history can be addressed through proactive wellness engagement. For example, home blood pressure monitoring can improve patient’s adherence and compliance to treatment. In addition, biometric data transmitted from remote patient monitoring devices with patient-specific thresholds increases the potential for early intervention. If elevated blood pressure and blood sugars are noted, quick engagement using telehealth can decrease the likelihood of acute episodes. Read data sheet >

Diabetes Remote Monitoring

According to the CDC, 26.9 million people, including 26.8 million adults, are diagnosed with diabetes, while 7.3 million people (21.4%) are undiagnosed. Zyter’s LTE-enabled glucose monitor not only helps diagnosed diabetics take charge of managing their condition, but can also help providers diagnose patients with other chronic illnesses who may be in the early stages of diabetes. Proactive intervention is key to halting or slowing the progression of the disease. The CDC also reports that 6 out of 10 persons who have diabetes also have high blood pressure. Diabetic patients are also challenged to maintain a healthy weight. All three Zyter RPM devices work together for helping patients control all additional risk factors to bring about better outcomes. Congestive Heart Failure Remote Monitoring

Over 6.2 million people in the United States are diagnosed with heart failure, with attributed risk to obesity, diabetes, and unhealthy lifestyles involving tobacco, high-sodium diets, and inactivity. Proactive changes in a patient’s lifestyle and consistent monitoring of blood pressure are obvious ways that patients with congestive heart failure (CHF) can have both improved physical and psychological treatment outcomes. Daily blood pressure monitoring with Zyter RPM provides a vital foundation for managing remote care for CHF patients, as well as gives the patient peace of mind that their provider can interact quickly if readings stray from an acceptable range.

But high blood pressure isn’t the only threat to CHF patients. Zyter RPM makes daily glucose monitoring easy, reducing the risk of diabetes as a complication that can worsen CHF. In addition, daily weight monitoring using the Zyter digital scale can support CHF patients and their caregivers to guard against obesity, which can exacerbate the patient’s physical limitations and breathing difficulties.  Read data sheet >

Maternal Health Remote Monitoring

Expectant mothers are at increased risk of developing high blood pressure and gestational diabetes. Obesity also increases the risk of birth defects, pre-term birth, and stillbirth. Proactive remote monitoring of blood pressure, glucose readings, and weight with Zyter RPM can help providers reduce these risks and provide expectant mothers with the care, support, and peace of mind they need as their pregnancy progresses.

For low-risk patients, using Zyter Telehealth and RPM improves access to care, reduces stress and time spent traveling, waiting, taking a reading, and making decisions with better and more frequent information. Read data sheet >

Obesity Remote Monitoring

The risk of cardiovascular disease is approximately 78% among men and women who are obese and have diabetes – a nearly 25% higher risk than for persons who are not obese. Obesity remote monitoring with Zyter’s digital weight scale helps providers track the patient’s weight, while also monitoring the accompanying risk factors of hypertension and diabetes with the other devices. In addition, providers can intervene quickly with a telehealth visit through the Zyter RPM dashboard when the patient’s readings spike out of their acceptable range. Read data sheet >

Covid 19

COVID-19 Remote Monitoring

RPM is a safe and useful method of care as the healthcare system works to combat COVID-19 and it reduces the risk of exposure. Two easily measurable signs of health – respiration rate and blood-oxygen saturation – are distinctly predictive of higher mortality. Anyone who receives a positive COVID-19 screening test can easily monitor for these two signs at home.
