Improving Care and Outcomes with Remote Patient Monitoring

Remote patient monitoring (RPM) is the 21st century office visit, and as we’re learning, a potential lifesaving modality of care. Over 6.2 million people in the United States are diagnosed with heart failure, with attributed risk to obesity, diabetes, and unhealthy lifestyles involving tobacco, high-sodium diets, and inactivity. Proactive changes in a patient’s lifestyle and consistent monitoring of blood pressure are obvious ways that patients with congestive heart failure (CHF) can have both improved physical and psychological treatment outcomes. Daily blood pressure monitoring with Zyter RPM provides a vital foundation for managing remote care for CHF patients, as well as gives the patient peace of mind that their provider can interact quickly if readings stray from an acceptable range.
