After gallbladder surgery, a 66-year-old male patient is recovering at home. A week later, he is readmitted with a serious post-surgical infection – a poor outcome for the patient, and a strike against the surgeon in the payer’s value-based care program. The surgery was clinically successful, so what went wrong? The patient’s care management coordinator learns that the patient had not filled the prescription for his post-surgery medication because he could not afford it. Had the care team known about this, both the physician and care coordinator could have collaborated to intervene sooner.

Identifying at-risk patients and intervening to prevent costly ER visits and readmissions is a key challenge for physicians in value-based care reimbursement programs. The integration of ZyterHealth™ with TruCare™ enables physicians in value-based care programs and care management coordinators to easily access all of the patient’s data on a single, secure collaboration platform. By being able to access both physiological and social data, care teams can identify at-risk patients faster and collaborate to uncover and solve the patient’s issue before it becomes an emergency.

For example, after logging into the integrated TruCare™ solution, a care management coordinator can quickly scan a list of patients on the My Dashboard screen for those most at risk. They identify a member who is a mental health patient having issues with getting the proper nutrition, and clicks over to that member’s clinical Care Plan screen containing RPM and other clinical data from integrated Zyter solutions.

Directly from the Care Plan screen, the care management coordinator can initiate a secure “provider chat” to request a dietary consultation for that patient. They can also view other medical analytics on the patient on a Key Metrics screen to identify any gaps in care that could put the member at even greater risk for a relapse, and take action to reach out to the patient. In turn, physicians can easily click on the integrated TruCare application through the ZyterHealth RPM dashboard to discuss the patient’s social issues with the care coordinator via secure messages, video chat, or emails.

With this comprehensive knowledge of the patient’s medical issues and social challenges, care coordinators and physicians can collaborate on the same technology platform to bring about better patient outcomes and satisfaction at lower costs, and ultimately fulfill the goals of value-based care.
